I hereby submit this application for a permit to transport the shipment from the port/ quarantine station and to keep it stored in the refrigeration storeroom/ warehouse until obtaining a permit to transport the shipment to Israel:

I hereby abstain from marketing or transporting the goods contained in the shipments from the above-mentioned location before receiving the permit to enter the shipments to Israel or to obtain another permit issued by you.

 

Further, I am prepared to have every imported shipment sampled by the representative of the veterinary border inspection center as a prerequisite for receiving the permit to transport the shipments to Israel.

  • Valid import permit with relevant documents indicated in the permit;
  • A copy of the gate pass at Ben Gurion Airport;
  • A copy of customs approval (if it is approved);
  • Commercial invoice;
  • Packing list;
  • Bill of landing;
  • Original hygiene certificate only;
  • A diagram of temperatures in the container (except for the fresh fish shipments);
  • Receipt of payment;
  • A form of the product / products label (scanned labels from outside Israel may be used, or scanned concrete labels that were on the shipment that arrived at the quarantine station);
  • License number of transporter with refrigeration license and valid standardization permit ______________________

I hereby declare that the information for issuing a permit to transport the shipment from the port to the refrigeration storerooms in Israel is correct and true. This permit is not considered a permit to market the products.

To use the shipment (marketing / production), a permit from the veterinary unit where the shipment was stored must be obtained.
